You will need a SDAT number and W-9 for your doula business.
If you are enrolling as a solo practitioner or a group practice, you’ll need to obtain a SDAT number by registering your business with the State Department of Assessments and Taxation (SDAT), and be in “Good Standing” with the State. Once you register your business with the state of Maryland, you will get an SDAT number. This number is required to be reimbursed by Medicaid.

If you don't want to start a doula business in Maryland, and you just want to work as a doula for someone else's business or doula group, you would be considered a "rendering provider". In this case, you do not need an SDAT number. You would just need to work under an already approved doula group.
You will need to submit a W-9 form with your Federal Tax ID. If applying as an individual (not as a business), this can be your Social Security Number (SSN). If a group practice or a sole proprietor, this will be your Taxpayer Identification Number (TIN) or Employment Identification Number (EIN). Individual billers can choose to use a TIN/EIN for payments if they want.
